At our Elevate Programme Launch last week, the Elevate team got to meet with the new cohort of mentees for 2025-26.
The Elevate Community Mentoring & Grants Programme 2025–2026, led by the Community Development and Health Network (CDHN) in partnership with the Public Health Agency (PHA), is now in its 7th year and continues to support grassroots efforts to tackle health inequalities across Northern Ireland.

The Elevate Team within CDHN will work in partnership with experienced mentors across the region to support the mentee groups through project development, delivery, and evaluation. Our partner mentor organisations for 2025-26 are ARC Healthy Living Centre, Mid and East Antrim Agewell Partnership (MEAAP), Supporting Communities and WOMEN’STEC.
This year’s successful groups have demonstrated how they will use a community development approach to address the social determinants of health in their application. A diverse range of community groups have been selected, and they are working on projects that address rural isolation, mental health, support for marginalised women, and engagement with ethnic minority youth to name a few.
